To find something infested with fleas for Betty in Kingdom Come: Deliverance 2, follow these steps as part of the "A Good Scrub" and subsequent "Ill Repute" quests:

Pick Up 'A Good Scrub'

Begin by speaking to the innkeeper in Kuttenberg about work, who will direct you to Betty at her bathhouse located in the southeast part of the city. Tell Betty you heard she has work and agree to go to Grund to find Matsek. Matsek is hesitant to be seen with the girls he's supposed to bring to the bathhouse. Approach the three girls at a table in the courtyard and inform them you're there for Betty, then escort them back to Kuttenberg.

Promote the Bathhouse

Upon returning with the girls, Betty will ask you to help spread the word about the bathhouse. Work on the promotional spiel with her, then shout it at the grain market, horse market, vintners’ street, and merchants’ street during the day.

Clean Up the Bathhouse and Finish 'A Good Scrub'

Next, you must clean the bathhouse before the alderman's inspection. Completing this task quickly is crucial to avoid the bathhouse being shut down, which would prevent you from picking up "Ill Repute" from Betty.

Finding Fleas for 'Ill Repute' in Kingdom Come: Deliverance 2

After passing the inspection, speak to Betty about ongoing issues. She will explain that the council is deciding on the main bathhouse of the city. To influence this decision, you need to make the councillor leave the competing bathhouse. Gather information on what he dislikes by speaking with Meaty Mary, Katcha, and Little Lida.

To proceed, you'll need Chamomile Brew to add to the wine cask on the second floor of the competing bathhouse. Little Lida will reveal that the councillor likes fragrant oils if you give her a noblewomen's dress from a tailor. Alternatively, sabotage by lockpicking a cabinet on the third floor to steal the oils.

Katcha will mention that the bathhouse used to have fleas. For the flea-infested item, head to the east gate of Kuttenberg City, where you'll find a small camp of beggars. Search the sacks there to find a flea-infested blanket.

Once you have the fleas, return to the bathhouse and go to the room where you added the Chamomile Brew to the wine. When no one is watching, interact with the baskets and pour the fleas into Councillor Naaz’s laundry basket and the clean laundry basket.

If you struggle to find the flea-infested item, remember that completing just one of the three tasks (Chamomile Brew, fragrant oils, or fleas) is enough to make the councillor leave, allowing you to finish the quest and move on to others like "Axe from the Lake."
