游戏背景设定在三国时期。玩家扮演一位对三国时代充满热情的女孩——Ayoung Cho。某天醒来，她发现自己身处三国乱世之中，正值汉朝末年，黄巾军四处作乱。
玩家将与关羽、吕布、赵云等传奇武将相遇，亲身参与历史战役，组建属于自己的终极武将阵容。这些武将拥有独特的AOE技能，主角甚至可以装备六种主动技能。
《Otherworld Three Kingdoms》的协同系统非常有趣。首先，你和你的武将之间存在成长协同效应。训练他们，你会发现你们的属性会相互叠加，使你的队伍更加强大。然后是阵型协同效应。有些角色拥有特殊的阵型技能，当你把他们加入你的队伍时，会获得额外的效果。
